결론 및 제언
류마티스 관절염의 피로는 대부분의 환자가 경험하는 것이기 때문에 이에 대한 연구가 필
요하다. 특히 질병 상태 이해와 임상적 간호중재를 위해 피로의 영향 요인 파악은 매우 필
요하다. 류마티스 관절염은 여성에서의 발병률이 남성에 비해 훨씬 높고 여성환자의 피로가
남성보다 심하며 한국 환자가 미국 환자보다 우울이 심하기 때문에 본 연구에서는 한국 여
성 류마티스 관절염 환자의 피로에의 영향 요인을 규명하고자 연구를 시도하였다. 미국 류
마티스 학회의 1987년의 기준에 의한 류마티스 관절염 환자 중 여성환자 124명을 대상으로
자료를 수집하였으며, 다음과 같은 결론을 얻었다.
류마티스 관절염 환자의 대부분이 피로를 느끼며 피로를 다차원적 피로척도로 측정하였을
때 중간이상의 점수를 나타내었다. 피로는 통증정도, 통증부위, 기능장애, 우울, 수면의 질과
정적 상관관계를 나타내었고 피로를 설명하는 주요 변수로는 통증, 우울, 수면의 질이었다.
본 연구결과와 함께 여성 류마티스 관절염 환자의 피로 영향요인을 인과론적으로 규명하
기 위해 피로 구조모형을 구축하고 이를 검증할 필요가 있다. 그러면 류마티스 관절염 환자
가 경험하는 피로의 인과적 경로를 이해할 수 있기 때문에 피로의 기전을 보다 확실하게 설
명할 수 있을 뿐만 아니라 환자의 피로를 감소시키는 간호중재의 개발이 가능하게 될 것이
다.

The purposes of this study were to identify the prevalence of fatigue, and factors
influencing fatigue in women with rheumatoid arthritis. The subjects were 124 patients
with rheumatoid arthritis. Most of all patients felt fatigue and mean score measured by
Multidimensional Assessment of Fatigue was relatively high. Fatigue was positively
correlated with pain intensity, the number of pain sites, functional disability, do pression,
and sleep quality.
A hierachical regression model was used to determine the variance which accounts for
fatigue. Pain intensity, depression, sleep quality accounted for fatigue significantly. This
finding indicates that pain influences fatigue through depression and sleep quality ;
depression through sleep quality. The causative factors could be identified by further
study of structural eqation model.
